Designed for seniors aged 60 years and above, Lifestyle Reshaping is a core health resource program that provides a personalised health promotion approach based on the individual’s health needs, living circumstances, past experiences, and strengths.
This comprehensive and person-centric program comprises:
- 10 weekly group and individual learning sessions covering
- Various specific health topics
- Personalised lifestyle reshaping and goal setting through individual/home visit sessions
- Introduction to easy-to-prepare nutritious snacks
- Introduction to simple functional fitness exercises
- Pre and Post program evaluations
- 2-4 sessions of assessments
- 2 follow-up group sessions, alumni gatherings
塑造健康生活是一个为期10周的健康老龄化课程,有小组学习活动和个人访谈。每周课堂会涵盖一个特定的健康主题。例如,老龄化过程、心理健康、健康饮食、失智症、预防跌倒、管理疾病,为临终事项做好准备、等等。课堂同时结合适宜的运动和身体机能锻炼,以及营养小吃分享。
《塑造健康生活》这整个项目的持续时间为1年。在10周的课程结束后,我们为毕业生组织2次聚会。我们也将与您进行至少2次健康访问/测量,以便全面了解您的生活习惯与整体健康需求和设定个人健康生活目标。
